New Patients 

Before scheduling your first appointment, we ask all new patients to complete our brief intake form. This form helps us understand your current needs, symptoms, and treatment goals so we can determine whether our clinic is the right fit for you.

Once your intake form has been reviewed and it appears that our services may be a good fit, a member of our team will reach out with the next steps. At that time, you will receive additional onboarding forms and instructions to access our secure patient portal, where you will be able to self-schedule your initial appointment.

 

This process allows us to provide thoughtful, personalized care and ensure that each patient is connected with the right level of support from the very beginning.

Once you have been approved for scheduling, you will receive access to our patient portal. If you carry one of the following insurance plans, please select the appropriate scheduling link provided below to create your account and book your appointment.
